Registration for 2012-2013 school year A nonrefundable $30.00 registration fee should accompany your completed and signed registration form. Two health forms are also required for enrollment--the immunization form may be filled out and signed by the parent. The Health Care Summary must be completed and signed by your clinic. These forms must be on file no later than the first day of class (we prefer that they be handed in at the time of registration). Please note that children MUST BE ABLE TO USE THE TOILET AND REDRESS INDEPENDENTLY before the first class session. All necessary forms are available for download using the links on this page.
